Liquid Personnel is currently recruiting for a Social Worker to join a busy Older People Team based in St Helens.

Your main responsibilities as a Social Worker:

  • To assess and care manage the complex needs of older people and their carers to ensure their health, wellbeing and social inclusion.
  • To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
  • To promote partnership working which supports the person centered outcomes to be delivered for older people.
  • To undertake safeguarding investigations as required.

What we are looking for in a Social Worker:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
