This Team needs your knowledge and expertise. We are currently recruiting for proactive social work practitioners to join our Colchester North/ South Neighbourhood Team in Colchester.

You will join a Team of forward-thinking practitioners and a leadership team committed to best practice. We have a mantra of "Start Right to End Right".

About the Role:

Adult Social Care, through effective practice, has made a transformational shift from a focus on long-term care support, to a model with four key principles: Prevention, Early Intervention, Enablement and Safeguarding.

Social workers manage a varied caseload in the community undertaking assessments, and reviews of commissioned services. Purposeful case management intervention through direct work with individuals building relations and applying various social work models and tools to support engagement with individuals, sign posting and working with health and other key partners.

This approach will enable individuals, their families and carers to access information and tools independently and for our function to respond proactively to growing service demand in a sustainable way, within the financial constraints of the Council.

Accountabilities:

  • Assess social care needs, undertake safeguarding Enquires and risk assessments. Undertake and assessments in line with LA duty of working in partnership with local ICBs in line with NHS CHC framework and Care Act.
  • Develop support plans for those individuals assessed as having eligible social care needs that drive the care required for the individual's needs.
  • Accountable for practicing within the statutory legal frameworks, policy, and practice guidance.
  • Working collaboratively building professional and effective relationships with internal and external partners, stakeholders, and the voluntary sectors.
  • Conducting triage and plan responses to safeguard a child or vulnerable adult include intervention as required.
  • Pro-actively influencing a range of planning, decision making, and case discussions inter-agency linked to Service/Team function(s).
  • Practice in a way which supports the Council's wider strategic aims.

About You

  • Educated to degree level i.e. Diploma or degree in social work and current Social Work England registration with post qualifying experience in Adult Social Care
  • Evidence of continuing professional development and demonstrable capability of practice in accordance with current professional standards e.g. professional capability framework, knowledge and skills statement.
  • Experience of work in Local Authority social care settings and in different operational Teams.
  • Commitment and passion driven by social justice, self-awareness, and desire to engage honestly and clearly with individuals whilst having courage conversations.
  • Understand the financial implications of care packages commissioned.
  • Evidence of clear decision associated with intervention.
