Community working, Stroke Experience
Location: Hull
Start date- ASAP
End date- 5+ months
Shift Pattern:
- Full or Part time
- 8:00 - 4:00 (Mon - Fri)
Skills/Experience:
- Community based to work on Early Supported Discharge.
- Need to be qualified not necessarily Post Grad 3yrs
- System one desirable but not essential
- Valid UK Driving Licence
- Must be self motivated and aspirational, be able to assess in an effective and timely manner in a rapid paced environment. Pathways will support early recognition of deterioration with appropriate escalation processes in place to maintain patient safety
- Support will be provided to enhance operational management skills and the post holder will contribute to service planning, development and teaching
