Care Navigator

Elliott Hall Medical Centre is a long established NHS GP practice within the borough of Harrow. We have 11,400 + registered patients.

An exciting opportunity has arisen for a part time Care Navigator join our friendly and supportive team.

Working hours : 16.5 - 22.5 PW and overtime when available

You will be the first point of contact for patients, visitors and supporting practice team. You will need to have the skills to resolve and manage issues that arise and be able to prioritise your workload, be flexible and have an excellent communication skills, good IT skills, attention to detail, be reliable, organised, trustworthy have excellent customer service skills, caring and empathetic manner are essential. Experience in customer service role, office administrator role and previous NHS employment is an advantage but not essential.

The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:

a. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition

b. Answer incoming phone calls, transferring calls or dealing with the callers' requests appropriately

c. Process patient requests for appointments

d. Initiate contact with and respond to requests from patients, team members and external agencies

e. Enter read-code data on Emis

f. Photocopy documentation as required

g. Data entry of permanent and temporary registrations and relevant patient information as required. The processing of changes to patient registration, deduction of record and new patient registration

h. Input data into the patients' healthcare records as necessary

i. Direct requests for information, e.g. SAR, insurance/solicitors' letters and DVLA forms, to the appropriate team

j. Manage all queries as necessary in an efficient and polite manner

k. Carry out system searches as requested

l. Maintain a clean, tidy, effective working area at all times

m. Monitor and maintain the reception area and noticeboards

n. Support all clinical staff with general tasks as requested

o. Participate in practice audit as directed by the audit lead

p. Support administrative staff, providing cover during staff absences

q. Action incoming emails when necessary

r. Scan patient-related documentation and attach scanned documents to patients' healthcare records

s. Complete opening and closing procedures in accordance with the duty rota

t. Support clinical pharmacist in the management of repeat prescriptions, ensuring that they are processed accurately and efficiently

u. Monitor stationery supplies

v. Manage the movement of patient medical records

w. Attend small group and team meetings

x. Replenish supplies in office, at front desk, kitchen and consulting rooms as requested.

y. Medical Record Envelope (MRE) management

z. Support the surgery
