Skills/Experience:
- Experience as an OT working in assessment and discharge planning in a busy acute medical environment
- Postholder will provide high quality and evidence based occupational therapy assessment and interventions, both group and individual, to people who access services and their carers/supporters/family members, in line with the occupational therapy pathways.
- Assessment of patients requiring rehabilitation in their homes.
- Rapid response for admission.
- Moving and handling risk assessment, prescription of equipment.
- The role requires strong moving and handling competency and ability to adapt flexibly to rapid response requests.
- Ability to liaise with social services / continuing health care/ equipment providers and the MDT.
- The role will be split between 3 locations and full time is desirable.
Must be willing to work at all 3 locations.
