Our client is looking for someone to join their trusted team! You'll work with Adult Social Care staff to support customers being discharged home from hospital to live safe in their homes and communities.

Job requirements

  • Help customers return safely home from hospital by completing property inspections, collecting belongings, dropping off food parcels and arranging quotes for deep cleans so discharge can go ahead.
  • Carry out holistic assessments with customers using an action plan to ensure you support the customer and help them to stay happy, safe, and well in their home.
  • Complete routine or one-off visits to people identified in the community as needing further support. Checking homes are safe and warm, completing referrals to support services, and checking essentials like food and medication are available.

What they're looking for:

  • No qualifications needed and full training will be given.
  • Experience of working with older or vulnerable people (desirable, not essential).
  • Knowledge of local resources and support services available for older people.
  • A genuine, honest, and empathetic approach.
  • Confidence and resilience to adapt quickly.
  • Someone who's self-motivated and can work independently, while also working well as part of a team.
  • Excellent communication skills.
  • An understanding of what great customer service is.
  • Ability to work under pressure.
  • Excellent ICT skills.
  • Excellent administration skills.
  • A full UK driving licence suitable for business use.
  • Weekend availability (able to work 1 in 4 weekends).
