Care Coordinator
This role will be mainly office based, but there will be occasions when you will be required to participate in the on call service and be required to visit our service users. Working closely with the Registered Manager, Team Leader and care team, your role will be pivotal in ensuring that not only is care maintained along with service excellence, but that all care staff are fully utilised and supported with the efficient organisation of care runs and rota allocation.
Your primary function will be to design and implement the care team rotas, to ensure we maximise the efficient and effective delivery of care to our clients. The care team should always feel valued and important to our delivery of care, and your position must maintain and improve this vital component of our operating strategy. It will be your responsibility to ensure we have the right number and quality of staff, and with the support of the internal recruitment team you will be responsible for arranging and carrying out interviews and checking candidate compliance.
Essential that you...

  • Have NVQ/QCF L2 and/or L3 diploma in Health and Social Care
  • Have at least 2 years of care experience in health and social care sector
  • Hold a UK full driving licence and have access to your own vehicle
  • Able to demonstrate ability to communicate information and develop individuals to improve their knowledge and skill set
  • Continually look to improve customer satisfaction and minimise customer dissatisfaction
  • Maintain effective relationships with field staff, colleagues and key stake- holders to ensure the effective and safe delivery of services.
  • have a good IT and communication skills
  • participate in our 'on-call' and 'back-up' during out of hours.
