An exciting opportunity has arisen for a Band 6 Specialist Occupational Therapist to work within the community Stroke and Neuro Service within the trust. The purpose of the post is to deliver specialist assessment and treatment for patients being discharged from the acute trusts following a new stroke.

Description:

The successful candidate will have a wide range of clinical experience across the stroke and neuro care pathway and an understanding of collaborative working across multiple agencies. They will have a passion for delivering pathways of care that meet both high clinical standards and the needs of individual patients and their families and carers.

Role Responsibilities:

  • To provide a specialist occupational therapy service to patients recently diagnosed with a stroke or neurological condition in a range of community settings using developed specialist practice underpinned by theoretical knowledge.
  • To be a specialist resource of information for health and social care staff, patients, families, carers, and external agencies who may require advice, support, and guidance in the management of stroke and neurological conditions.
  • To transfer individual patients' ongoing care to other agencies as appropriate and ensure that the service provided is conducted in a professional, efficient, and effective way.
  • To meet the service specification objectives and key performance indicators as part of a multidisciplinary team.
  • Provide a comprehensive specialist assessment of patients who are referred to the service, often with complex presentations, in a holistic manner using investigative and analytical skills.
  • Apply advanced clinical reasoning to devise effective rehabilitation treatment plans, seeking guidance from the senior occupational therapist where applicable.

Skills:

  • Strong clinical skills in assessing and treating a range of conditions.
  • Ability to develop and implement effective rehabilitation and discharge plans.
  • Excellent communication and teamwork abilities.
  • Utilizes and interprets standardized outcome measures relevant to stroke/neuro with support from Band 7 where necessary.
  • Demonstrates evidence of recent CPD and an ability to reflect on experiences.
  • Experience with community-based pathways is advantageous.
