Community Care Officer

Seven Social Care are currently recruiting for a Community Care Officer to start a new role in Nottingham.

As a Community Care Officer, you will complete Care Act assessments: these are both face to face or telephone assessments, depending on the level of risk and the needs of the citizen being assessed, with consideration of mental capacity throughout.

You will work with supportive colleagues, citizens, carers, statutory and community partners to provide appropriate support that will enable independence, choice, and control, promoting early intervention and local community led support services to enhance well-being and respond to local community needs.

Responsibilities:

  • To ensure the delivery of appropriate services to safeguard vulnerable adults and their Families, according to their assessed needs.
  • To undertake all levels of Assessments in relation to the needs of adults and their carers against the departments agreed eligibility criteria and in accordance with National Performance Indicators within set timescales.
  • To create and implement culturally appropriate care plans and services that demonstrate Flexibility and innovative use of resources and undertaking a care coordination role.
  • To undertake case management responsibilities that include proactively managing a caseload 9including elements of risk management), the monitoring, Co-ordinating and Review of care plans and ongoing service provision.

Applicant Requirements:

  • Experience: Experience working with adults in a social care/health/voluntary sector setting.
  • Qualifications: Level 3 NVQ in Social/Healthcare.
  • Compliance: Adults Enhanced DBS, Right to Work in the UK
  • Hours: 37 hours a week, Monday to Friday, hybrid
  • Travel: Full UK driving license and access to a vehicle
