Support Coordinator Opportunity at Harrow Council
Pertemps is excited to offer a rewarding opportunity for a self-driven and motivated Support Coordinator to join our expanding team. As a Support Coordinator, you'll play a vital role in enhancing the lives of tenants and prospective tenants.
Key Responsibilities:
1. Compliance: Adhere to Sheltered Housing policies and procedures, including those related to Lone Working, Home Visits, and Support Delivery.
2. Collaboration: Work constructively with colleagues, providing cover for absent team members and actively participating in training and staff meetings.
3. Case Management: Effectively manage a caseload, ensuring timely case closure.
4. Complex Support: Address a range of complex support needs and risks that significantly impact tenants.
5. Well-being Monitoring: Regularly monitor tenant well-being through daily or weekly contact.
6. On-Site Support: Participate in a weekly rota to provide on-site support surgeries at each housing scheme.
7. Assessments: Conduct needs and risk assessments for prospective tenants, visiting applicants' homes.
Qualifications and Skills:
1. Support Knowledge: Familiarity with support delivery practices.
2. Risk Management: Working knowledge of risk assessments and risk management.
3. Experience: Previous work with elderly and/or vulnerable populations.
4. Organizational Skills: Ability to prioritize workload and respond promptly to urgent situations.
5. Assessment Expertise: Experience in developing needs and risk assessments, as well as support plans.
