Social Care Assessor

Wrexham Council are seeking a Social Care Assessor to work alongside Social Workers in providing information, advice, and assistance as the first point of contact for children in Wrexham Children's services. The successful candidate will be responsible for undertaking What Matters conversations with parents, carers and children, liaising with partner agencies across the statutory, public and voluntary sector, assessing care and support needs, and working with families to assist with improving their wellbeing.

Key Responsibilities

  • To carry a caseload of non-complex cases or to assist with complex cases, assessing needs, contributing to the formulation of plans, implementing and contributing to reviews of those plans, and carrying out such tasks as may be necessary to implement the plans.
  • Liaising with partner agencies across the statutory, public and voluntary sector
  • To visit and work directly with service users in order to consult them on their preferences, establish their needs and to implement designated actions from care plans.
  • Working with families to assist with improving their wellbeing.
  • Providing information, advice, and assistance to children and families.

Requirements

  • NVQ Lvl 3 Promoting indp/Health & Social Care or equivalent experience.
  • Knowledge of the Social Services and Well Being (Wales) Act 2014.
  • Excellent communication and interpersonal skills.
