We are seeking a full-time Team Leader (Care Assessor) with a driver's license and access to a vehicle, working alongside families, health and care professionals and community groups to co-design care and support plans that meet individual outcomes.

Position: Team Leader (Care Assessor)

Location: Newcastle (Regent Centre)

Job Type: Full-time, permanent

Hours: 37.5hr per week (agreed shift patterns with weekend working on a rota basis)

Salary: £25,350 per annum and up to £2,600 through our generous on-call payments

Examples of this proactive approach include:

  • Offering wellbeing support, like spending additional time with a client who needs a comforting chat over a cup of tea
  • Assisting with community engagement by connecting individuals to local services
  • Setting up digital connections that enable clients to stay in touch with their loved ones.

You will be an engaging and caring communicator, co-designing care, and support plans with the people we support and their families, ensuring their personal care needs are met, dignity is maintained, and individual outcomes and preferences are reflected. You will take a positive risk-taking approach, reflecting this in risk assessments and supporting colleagues to deliver high-quality care.

By fostering strong collaborations within the team and with external organisations, you will be instrumental in creating personalised, impactful outcomes that resonate deeply with clients' lives.

About You:

This position requires a dedicated professional who is passionate about making a tangible difference through teamwork and innovative care strategies.

Experience, knowledge, skills and qualifications:

  • Experience in coaching, mentoring, or training colleagues in care settings or health and care tasks.
  • Excellent communicator, both written and verbal.
  • You must have a valid driving licence and have access to a vehicle for work purposes.
