Myn is recruiting for a long-term client in the South East of England. We are looking for an experienced Hospital Social Worker to join a dynamic and supportive team. The Role:
You will provide critical support to unqualified staff, conduct Care Act assessments, and work on a caseload of approximately 10-12 complex cases per hospital ward. You'll play a key role in attending ward MDTs (multidisciplinary team meetings), collaborating with other professionals, participating in best interest meetings, and discharge planning.
Key Responsibilities:
Caseload Management:
Handle a varied caseload of complex hospital-based cases, ensuring all assessments and care plans meet the needs of the individual.
Multidisciplinary Team Collaboration:
Work closely with healthcare professionals in the hospital, attending MDTs and supporting discharge planning.
Care Act Assessments:
Carry out thorough Care Act assessments to ensure the best outcomes for service users.
Support & Mentoring:
Assist and guide unqualified staff and participate in team meetings to support continuous development.
What We're Looking For:
Experience in Adult Social Care.
Hospital social work experience is highly desirable.
Driving Licence:
A valid UK driving licence and daily access to a car are essential.
Team Player:
Ability to work well in a friendly and supportive team environment with 22 staff members, including senior social workers and managers.
Strong Communication Skills:
Confident in engaging with professionals, service users, and families.
Interview Process:
- Mixed competency and strengths-based interview.
- Two interviewers via Microsoft Teams.
