Responsibilities:
- Manage duty desk within the Older Adult CMHT
- Complete assessments in the community following new referrals
- Hold a small caseload for care coordination
- Role is office based but includes home/care home visits to complete assessments (Flexi work can be neg)
To be considered for the Social Worker role, ideally you will have:
- 1-3 years experience of working within a similar setting
- Diploma/Degree in Social Work
- Registration with SWE
