The Service
Assertive Transition Service in an IMPACT Provider Collaborative East Midlands commissioned service. Our team covers the southern part of the East Midlands region (Northamptonshire and Leicestershire). This service is mandated to help expedite the safe discharge of forensic mental health patients from secure care into communities within the East Midlands, by providing intensive, and individualized support programmes that allow a smooth and successful reintegration into the community.
To do this, we have a dedicated multi-disciplinary team in place that works with and alongside existing support teams including mental health hospital staff, community forensic services, local mental health teams, substance misuse support services, local housing providers, plus other community agencies and third sector providers. The shared aim is to reduce lengths of hospital stay for patients, unblock any barriers to smooth and effective community step-down, bring support, confidence and self-help tools to patients that will help them settle in their communities, and intensively work with patients and all other stakeholders to minimize any relapses or need for hospital readmission.
The Role
This role will suit an experienced Mental Health Nurse/ Learning Disability Nurse/ Occupational Therapist or Social Worker of NHS band 5 or band 6 equivalent level. It will be an essential part of the MDT effort to prepare forensic patients for and support their transition from secure hospital into the community. The role will work with patient's pre, during and post discharge, helping them acquire the skills, knowledge and confidence to be able to settle, work in and make contributions to the community in which they will live.
The post holder will champion each case and navigate the patient's pathway out of secure care. They will contribute to the development of bespoke patient plans, work on the daily living skill needs of individual patients, interface between patients' clinical teams and community teams, contribute to the production of forensic risk assessments, liaise with other agencies and parties involved in the discharge process, and generally co-ordinate the strands of a patient's care that will need to come together for transition to be successful. Patient support and interventions will continue as intensively as necessary once the discharge has taken place until it is clear that the patient is settled and functioning well.
There will also be a need to play a part in a Crisis Response service during the working week and potentially occasionally at weekends/evenings.
About You
- You will be a professionally qualified and registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker, with experience of the needs of recovering forensic mental health hospital patients, as well as knowledge of community step-down and the stakeholders and agencies involved.
- Above all, you will be passionate about advocating for your patients and bringing together all people and resources that will allow their pathway to the community to be navigated quickly and successfully.
- You will need to have a driving license and access to a car for travel in Leicestershire and/or Northamptonshire, with occasional visits to Milton Keynes and Nottingham too. Car/travel expenses will be reimbursed.
