Adult's Social Worker
Team: Hospital Discharge

Rate: £35 per hour
An Adult's Senior Social Worker/ Advanced Practitioner is required for the Hospital Discharge Team in Lincolnshire.
Senior Social Workers required to work onsite at a busy Acute Hospital in Boston, Lincolnshire completing proportionate assessments to support a person's discharge from hospital in a timely manner.
Utilising Home first principles and strengths-based practice the workers would be supporting the individual, working with their families and carers, and liaising with multi-disciplinary teams to support discharge to the person's own home where possible.
Being able to work collaboratively with Health and the Voluntary sector is a must.
Knowledge of the Care Act, Mental Capacity Act, Continuing Healthcare and Safeguarding practice are essential to the role as workers for these positions will support junior members of the team.
This is a 7 day service - working as part of a rota at weekends is included in the job role.

- DIPSW or equivalent qualification in Social Work
- Social Work England registration
- Post qualification experience
- Enhanced and in date DBS certificate.
- Excellent communication skills and be able to make good decisions based on information available.
- One year minimum post qualified experience in Adult'sSocial Work
- Can work alone and within/contribute to a team/multi-agency set ups.
- Possess knowledge of service needs.
- Competent IT skills and be able to navigate electronic file/recording systems.
- Can write clear, concise, and robust assessment/reports/files.
- Have a flexible approach to work and working hours when the need arises
