Job title: Social Worker, Adults, East Doncaster

Job Type: Full time, Permanent

Hours: 37 hours per week

Salary: £31,372 - £41,438

Purpose:

As part of our team, you will have the opportunity to work with people who are in prison within Doncaster prisons, as well as oversee the applications to the Extra Care housing Schemes across the City of Doncaster.

Responsibilities and Duties:

Support people to talk about their strengths, interests and social circumstances, and connect them with local resources and agencies that can help them achieve their goals.

To conduct assessments of the needs and risks of adults in the community and prison, using a strength-based approach

To develop and review personalised care and support plans with the involvement of the person, their family and other relevant professionals

To provide information, advice and guidance to people and their families on the available services and resources in their local area

To liaise and collaborate with other agencies, such as health, housing, education, employment and voluntary sector, to ensure coordinated and holistic support for the person

To safeguard the rights and wellbeing of adults at risk of abuse or neglect, and to report any concerns to the appropriate authorities

To maintain accurate and up-to-date records of the work done with the person, using the relevant systems and tools

To participate in regular supervision, training and team meetings, and to contribute to the continuous improvement of the service

To adhere to the professional standards, codes of practice and ethical principles of social work.

Knowledge & Skills:

Experience of working with adults in a social care setting

Knowledge of relevant legislation, policies and procedures

Excellent communication and interpersonal skills

A commitment to person-centred and strength-based practice

A flexible and adaptable approach to work

A willingness to learn and develop your skills

Experience:

Relevant post-qualification experience in adults social work within a statutory, voluntary, or independent sector organisation.

Qualifications:

CQSW / DIPSW / Degree in Social Work or equivalent.

Special Conditions:

Registration with SWE

Must be a driver with own car
