Key Responsibilities:
- Conduct thorough assessments of children and young people with additional needs.
- Develop and implement personalized intervention plans.
- Provide training and upskilling to families, caregivers, and other professionals.
- Deliver effective and evidence-based interventions.
- Manage and efficiently close episodes of care.
- Contribute to the ongoing development and improvement of our services.
- Visit community sites, including schools and homes, to provide on-site support.
Essential Criteria:
- Experience working with children and young people with moderate to complex additional needs at a Band 7 level.
- Proven ability to assess, plan, implement, and evaluate interventions.
- Excellent time management skills and the ability to manage a caseload effectively.
- Strong communication and teamwork skills.
- A valid driving license and access to a car for community visits.
