Job Overview:
We are seeking a motivated and skilled Band 6 Community Occupational Therapist to join our dynamic team. This role involves managing your own caseload, providing meaningful rehabilitation in community settings, and making a positive impact on patients' lives. The role also offers opportunities to develop skills in triage, rapid response, and managing deteriorating and long-term conditions.

Key Responsibilities:

  • Provide holistic rehabilitation tailored to individual patient needs in community settings.
  • Manage your own caseload, delegating tasks to therapy assistants and others to optimize outcomes.
  • Work closely with multidisciplinary teams to deliver high-quality care.
  • Develop leadership skills by supporting therapy assistants, students, and peers.
  • Participate in clinical supervision, peer support, and continuing professional development (CPD).
  • Contribute to service improvement projects across the trust and wider healthcare partners.

Requirements:

  • Degree in Occupational Therapy and HCPC registration.
  • Experience working in community healthcare or similar settings.
  • Strong communication, leadership, and organizational skills.
  • Ability to work independently and collaboratively within multidisciplinary teams.
  • A commitment to professional development and improving patient outcomes.
