Key Responsibilities:
- Conducting comprehensive assessments of needs, strengths, and risks.
- Developing, implementing, and reviewing personalised care plans.
- Responding to safeguarding concerns and ensuring the protection of vulnerable adults.
- Managing a diverse caseload and maintaining accurate records.
- Coordinating services and working in partnership with health services, housing, and voluntary organisations.
- Staying informed of relevant legislation and best practices in social work.
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
-Eligibility to work in the UK
