Role Overview:
Conduct short breaks assessments to determine respite support needs for children with disabilities. Review existing short break packages to ensure they meet the child's needs and decide if support should continue. Perform home visits, school observations, and work collaboratively with multiple agencies. Complete carers assessments to support families effectively.
Requirements:
Driver preferable (or willing to travel around) for community home visits. Strong assessment and review skills. Ability to work in a multi-agency environment.
