Occupational Therapist

Are you an Occupational Therapist seeking a new challenge? Do you have previous experience supporting adults in a healthcare setting?
Service Care Solutions are working with North Lincolnshire Council to recruit a qualified Occupational Therapist to join the NLC Independent Living Team.

The successful candidate will assess equipment needs, make home adaptations, and support care home discharges, helping clients to live independently and safely.
Contract Duration: 5 August 2024 to 4 October 2024, with potential for extension
Hours: Full-time, 37 hours per week
Shifts: Monday to Friday, 09:00 - 17:00
Location: Scunthorpe
Pay: £30.00 p/h LTD

Key Responsibilities:

  1. Assessment and Implementation of Equipment and Adaptations:
  • Conduct thorough assessments of clients' needs to determine the appropriate equipment and home adaptations required to support independent living.

  • Develop and implement personalised care plans that include recommendations for assistive devices, home modifications, and other interventions to enhance clients' safety and autonomy.

  • Coordinate with suppliers and contractors to arrange for the timely provision and installation of necessary equipment and adaptations.

  1. Support for Care Home Discharges:
  • Facilitate the safe discharge of clients from care homes, ensuring that they have the necessary support and resources to live independently.

  • Collaborate with care home staff, healthcare professionals, and community services to create comprehensive discharge plans that address clients' physical, emotional, and social needs.

  • Monitor and evaluate the effectiveness of discharge plans, making adjustments as needed to ensure optimal outcomes for clients transitioning from care homes to independent living.

Essential Criteria:

  • Qualifications:

  • Degree in Occupational Therapy or equivalent.

  • HCPC registration.

  • Experience:

  • Proven experience in assessing and implementing equipment and home adaptations.

  • Experience facilitating safe discharges from care homes.
