About us:
This is an exciting opportunity to be part of a brand-new Adolescent Service that has been created in Surrey, bringing together a range services for young people including those who go missing, are involved in gang culture and are vulnerable to exploitation, including criminal and sexual exploitation. Our team includes Social Workers and Adolescent Practitioners who are focussed on achieving positive outcomes for young people.

About our Adolescent Practitioner roles:
The Adolescent Service is built on the belief that relationships are the key vehicle for achieving change. The service is underpinned by 'Relational Practice' principles, an intentional way of working where establishing, attending to, and maintaining empathetic and honest interpersonal relationships are prioritised.

This approach includes our relationships and how we work with each other. Adopting this approach supports us to create systems that can be trauma-informed and respond appropriately to individuals' different needs and experiences. It supports everyone's ability to work restoratively to resolve conflict, repair harm and support change.

All roles hold relational and restorative practice at their core; and roles in our Regional Hubs will practice across the broad range of adolescent needs, working in close partnership with social workers, other partner agencies and the voluntary sector to provide flexible, seamless access to frontline expertise.

Our Central Hub roles will focus on slightly more specialised areas of work, centred around exposure to extra-familial harm. Our Risk and Intelligence team will lead on the mapping and understanding of networks and concerns around exploitation and modern slavery and human trafficking, whilst our Intervention Support team are our contextual intervention specialists, working to understand and intervene with the contexts where children experience or are at risk of experiencing harm.

Adolescent Practitioners in our Central Hub Teams:

  • Deliver focused, specialised intervention, crisis response, context-based assessment and plans.
  • Build data and intelligence and support with mapping, Work closely with the Edge of Care service.
  • Develop expertise and knowledge across the practice system,
  • Consult and support on work to help across the whole practice system (from Assessment to Leaving Care), working directly with children, families, and other providers as required.
  • Ensure that case notes are adequately recorded as per the team policy and procedures, including timely and accurate case notes, assessments and reports

Adolescent Practitioners in our Regional Hub Teams:

  • Support whole families through a strengths-based and trauma informed approach, utilising motivational interviewing skills and effective relational approaches.
  • Coordinate key support services and professionals with families, to form an engaged, active and effective Team Around the Family with clearly defined roles and expectations as per the family plan.
  • Ensure casework is adequately recorded as per the team policy and procedures, including timely and accurate case notes, assessments and reports.
  • Are the lead practitioner for a caseload of families, maintaining positive contact and effectively managing the balance of providing hands-on support and coordinating relevant partners, as per a shared family plan, to keep them safe from harm.
