Assertive Outreach Worker - Bath & North East Somerset

RSR Housing are currently recruiting on behalf of our client based in Bath & North East Somerset for an Assertive Outreach Worker.

The purpose of this role is to deliver a high-quality person-centered outreach service, working as part of a team, contacting new, repeat and entrenched rough sleepers in Bath, implementing a locally agreed approach to No Second Night Out.

This is a part time (30 hours per week and participation in an on-call, out-of-hours rota), permanent role offering a salary of £19,600 per annuum.

Main Duties:

  • To undertake assertive outreach and hot spot visits across the locality area including during unsocial hours, maintaining a regular street presence
  • Identify, engage and assertively work with people sleeping rough within the locality areas
  • Carry out needs/risk assessments with identified rough sleepers to secure access to services which are suitable for their immediate needs, and make appropriate referrals in partnership with them to establish a sustainable life away from the streets as quickly as possible
  • To work within No Second Night Out guidelines when working with those new to the streets
  • Develop and implement person-centered packages of support with people sleeping rough
  • To work in close partnership with the local authorities, police, probation, emergency accommodation providers and wider partner agencies to maximise positive outcomes for Clients
  • To work closely with substance misuse services, mental health services, Probation, and HMP Eastwood Park to help prevent people being released from prison homeless.
  • To track the progress of the referrals made in respect of identified rough sleepers
  • To build relationships with people who are sleeping rough, using inter-agency outreach sessions and regular visits to agencies/venues visited by people who are rough sleeping
  • To give clients advice, information and provide an enabling service so that they can access appropriate services
  • To carry out risk assessments and complete Housing Benefit forms for rough sleepers moving into designated accommodation services
  • To recognise indications of substance misuse, mental health needs, personality disorder behavioural issues and make appropriate referrals to relevant agencies
  • To promote and facilitate effective multi-agency and joint working to address begging, street drinking and other street-based activity

Essential Requirements:

  • Have full driving license and access to own car.
  • Good knowledge and ability to use Microsoft Word, email and basic spread sheets.
  • Understanding of the causes of offending, homelessness and social exclusion and approaches which seek to address these.
  • Ability to undertake Needs Assessment and Planning for people with complex social needs which includes taking a lead on multi agency working and partnerships.
  • Previous work with homeless and/or other socially excluded people.
  • Working as part of a team in a social or health care setting.
  • Working with people with complex needs and challenging behaviour.
  • Experience producing accurate reports and data as requested and at specified intervals.

Role is subject to DBS check at the relevant level.
