Key Responsibilities:

  • Conduct comprehensive assessments of new clients to identify their care needs and develop tailored care plans.
  • Review and update existing care plans to ensure they meet client's evolving needs
  • Coordinate and schedule care services for clients, ensuring optimal coverage and timely delivery of care.
  • Communicate effectively with care staff to allocate tasks and ensure clarity in service delivery.
  • Provide support and guidance to care staff, fostering a collaborative and supportive working environment.
  • Conduct regular team meetings and training sessions to address any issues and ensure high standards of care.
  • Act as the primary point of contact for clients and their families, addressing any concerns or queries promptly and professionally.
  • Foster strong relationships with clients and their families to ensure satisfaction with our services.
  • Ensure all care activities comply with relevant regulations and best practice guidelines.
  • Maintain accurate and up-to-date records of client care plans, assessments, and service delivery.
  • Respond to emergencies and unforeseen circumstances effectively, ensuring the safety and well-being of clients and staff.

Profile

Requirements:

  • Previous experience in a care coordination or similar role within the health and social care sector.
  • Strong understanding of care planning, assessment, and delivery processes.
  • Excellent organisational and time-management skills.
  • Compassionate and empathetic nature, with a genuine commitment to improving the lives of clients.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and care staff.
  • Knowledge of relevant regulations and best practice guidelines in the care sector.
  • Proficiency in using care management software and Microsoft Office applications.
