Southwark
Our Local Authority client in Southwark are seeking an Occupational Therapist to support their Adult Social Care Team.
The Local Authority are currently piloting a dedicated care home therapy service that focuses on supporting the discharge pathway from hospital to care homes in Southwark. This service will cover the 7 larger care homes as well as input into a few of the smaller care homes in the borough.
Responsibilities:
- You will be required to help facilitate effective working practices in the care homes by providing coaching, advice and joint working.
- Take a key role in the implementation and evaluation of this new care home service, in conjunction with partners from the NHS, primary care and social care.
Requirements:
- HCPC registered Occupational Therapist with at least one recognised OT qualification.
- Significant experience of delivering high quality case management to older adults, specifically with dementia to live well through identifying and addressing care and therapy needs.
- Previous experience in posture and seating assessment and management will be a bonus.
