Social Worker - Hospital Team
South Wales based local authority are currently seeking a Locum Qualified Social Worker to join their team within the Initial Support Service. This role focuses on supporting both hospital discharges and community referrals.
Key responsibilities include:
- Conducting initial assessments, care plans, and reviews.
- Participating in MCA (Mental Capacity Act), Best Interests (BI), and DST (Decision Support Tool) meetings.
- Planning support for individuals in their own homes.
- Assessing eligibility for care placements.
The team holds cases for a maximum of 12 weeks, with a fast turnover, allowing you to complete tasks efficiently and move cases forward without long-term involvement.
