Description
- Working as part of the wider MDT
- Supporting the At Home and Assisted Discharge Team
- Supporting services users who are being discharged from hospital into their own homes
- Complete and Follow Support Plans
- Provide practical and emotional support
- Based from one site in PE3 with visits in the Peterborough area to support service users in their own home
Profile
- Experienced as a Support Worker
- Can work as part of a team but also be confident working alone
- Basic IT Skills
- CAR DRIVER IS ESSENTIAL FOR THIS ROLE
- Client will arrange for you to complete their mandatory training
- This role is subject to an Enhanced Adult DBS (which can be applied for if required)
Job Offer
- Monday to Friday 10am - 6pm
- Occasional weekends
- 6 Week Contract which may be extended
- Starting 7 October
- £12/Hour paid weekly
