Our client, Nottinghamshire County Council, is seeking a Social Worker for its Joint Funding Review team.
The role will involve the following:
- Joint funded support
- Continuing Healthcare (CHC)
- Decision support tool meetings (DST)
- Strengths based approach
- Joint working with health colleagues
- Negotiation skills needed
Requirements of the role:
- Qualified Social Worker
- Fully Social Work England registered
- Must have strong post-qualified experience working in an adult social care setting
- Must have excellent knowledge and experience with CHC
