Location: North West London
This position is available for an experienced band 6 health visitor, who will be working with vulnerable children and their families in the community.
Start date: ASAP-3 month minimum
Shift pattern: Monday-Friday, 9:00-17:00
Hours: Full time (37.5hrs/week)
Job description:
- Leading and coordinating the healthy child programme by carrying out the 5 mandated contact visits.
- Providing a seamless service to universal and vulnerable children and families by working in partnership with multidisciplinary teams.
- Early identification, intervention, and appropriate onward referral.
