An excellent opportunity has arisen for a Mental Health Nurse to join our Recovery & Wellbeing Team. You will work as part of a multi-disciplinary team and provide specialist assessment, treatment and care to a caseload of people with a range of mental health problems from the local community. You must be enthusiastic, self-motivated, flexible and able to work as part of a multidisciplinary team and demonstrate a commitment to the recovery model. Workers will need to visit service users in the community, and will need their own transport.
Key Responsibilities
Working autonomously as a Lead Practitioner to a defined caseload of clients. This will include offering specific interventions to people diagnosed with mood/anxiety disorders, psychosis or complex emotional difficulties.
You will complete initial assessments and work closely with our referrals co-ordinators.
To undertake assessments of individuals and their families in the designated care group
with complex health presentations, including those service users presenting with higher
levels of risk. To develop alongside the service user their care plan.
To offer specialist clinical assessment and advice where required.
To assess and manage on going risks as identified during the assessment, ensuring that
this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.
To participate in the planning, development and evaluation of profession specific and local
multi-professional services.
To ensure that clinical practice is evidence based and consistent with relevant NICE
guidelines and Trust policies and procedures.
To provide supervision of junior staff and trainees where appropriate.
To act within the relevant professional code of conduct at all times
