Care Coordinator

As the Care Coordinator, you will be responsible for the overall management and scheduling of client rotas and will participate in the on-call rota. With outstanding planning and organisational skills, you will ensure care resources are used efficiently, using excellent communication skills throughout all call planning and delivery. You will have a positive attitude and lots of energy, using innovation in your problemsolving. Main Responsibilities :

  • Planning and organizing the community care rotas.
  • Undertaking initial care assessments, producing care plans, and reviewing these on a regular basis
  • Undertaking supervision of staff as per the supervision schedule
  • Taking part in an evening and weekend, out-of-hours emergency rota (including covering care calls when required)
  • Dealing with new client enquiries
  • Ensuring regular communication with clients and care staff
  • Dealing with client and care staff enquiries and queries
  • Ensuring and monitoring quality control of the service provided and the implementation and involvement of staff training programmes as and when required.
  • Participating and contributing to Team Meetings
  • When necessary, undertaking care calls to ensure knowledge and experience is maintained and to cover emergencies while on-call
  • Undertaking mandatory and optional training to ensure knowledge and skills are developed.
  • Care Coordinators may also be required to interview potential new carers and be involved with the supervision of staff.

What we'll need from you :

  • Solid understanding of daily domiciliary care requirements
  • Preferably holding a care or nursing qualification
  • Passionate about client service
  • Team player who can also work independently
  • Strong verbal and non-verbal communication skills
  • Good time management and prioritization skills

  • Demonstrated supervisory skills

  • Punctual and able to meet travel requirements

This role involves working in regulated activities with vulnerable groups and as such, is subject to a DBS disclosure.
