To work within the Upper Lea Valley Community Team/Lower Lea Valley ICT Team across the sites including Hoddesdon Health Centre and Bishops College.
Experienced Occupational Therapist to provide specialist Occupational therapy to patients in the community.
Car driver essential
Be part of an integrated health care team; to provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital, support early hospital discharge and work with a core community caseload.
- To offer brief (6 sessions) interventions for energy management, e.g. activity baselines, pacing, rest breaks, relaxation, sleep hygiene, and goal setting
- Contribute the skills, knowledge and performance of the OT team
