**About the Role:

A Local Authority in Greater Manchester is seeking a highly skilled and experienced Social Worker to join their Disability Enablement Service, a specialised team dedicated to supporting service users aged 0-25 with a diverse range of complex needs. This role is integral to the Transitions Service, providing crucial support as young people move from childrens to adult services, ensuring a seamless and person-centred transition.

**Key Responsibilities:

  • Transitions Social Work: Work closely with young adults aged 18-25, supporting their transition from childrens to adult services. You will be responsible for conducting Care Act 2014 assessments, developing comprehensive support plans, and coordinating multidisciplinary input to meet the holistic needs of the service user.
  • Multi-Agency Collaboration: Engage regularly with professionals across health, education, and social care sectors. This role requires close collaboration with medical professionals and other agencies, ensuring that all clinical and social needs are addressed in a coordinated and effective manner.
  • Care Act 2014 Compliance: Apply your in-depth knowledge of the Care Act 2014 to assess eligibility for adult social care, create personalised care plans, and safeguard vulnerable service users. Your expertise will ensure that the rights and needs of individuals are upheld throughout the assessment and care planning process.
  • Family Safeguarding Model: The Local Authority has recently implemented the Family Safeguarding Model, focusing on a more holistic, family-centred approach to safeguarding. You will be expected to incorporate this model into your practice, ensuring that safeguarding interventions are comprehensive and inclusive of the entire family unit.
  • Supervision and Support: Benefit from regular, structured supervision, with weekly sessions to discuss caseloads, challenges, and professional development.
