Nurse Case Manager - Complex Care

An exciting new Nurse Case Manager opportunity has become available with a highly successful, independent Complex Care Agency operating across London and surrounding areas.

They are looking for a passionate and driven individual to focus on assessing new clients and ensuring carers are trained up to deliver the correct care to these individuals. You MUST be Nurse qualified for this role and previous experience with Tracheostomy and Ventilated Care is ideal.

Key Responsibilities of the Nurse Case Manager:

  • Go out in the community and carry out assessments on new clients
  • Assess the skills that are needed to treat that patient and then ensure the Carer can deliver the care
  • Train the Carer or advise the Trainer what training is required
  • Accompany the Carer to the visit guide them through it and then direct them through what they need to do
  • There will be regular travel in London – this is NOT an office based role

Nurse Case Manager Requirements:

  • Nurse qualified
  • Experience with Tracheostomy and Ventilated Care
  • Good medication abilities
  • Strong communication skills
  • Empathetic in nature
