The post holder is required to coordinate and conduct assessments to decide what care might be useful in empowering individuals to recover and remain well. The role involves provision of high quality interventions, care co-ordination, the monitoring and care planning for service user's physical health concerns as well as other tasks integral to the individual's recovery and wellbeing. The post holder will work collaboratively with other disciplines and other teams in the locality to meet the needs of the local dual diagnosis population.

Responsibilities

To coordinate and conduct assessments to a high standard demonstrating a high level of expertise in substance and alcohol related issues. To motivate, assess and refer patients to the appropriate treatment with compassion and empathy, using appropriate skills such as motivational interviewing, cognitive and behavioural approaches and considering psycho-social interventions.
Assessment and care will also involve physical health needs, monitoring and intervention within the post holder's and services' capabilities and competencies.
To involve carers in this process and to take into account their needs as well as those of the client.
To establish and maintain good working relationships with local agencies
Co-ordinate multi-agency activity for clients with complex needs involving working closely with other BHFT and Wokingham Borough Council services such as the crisis team and housing.
Organise own caseload and prioritisation by need including audit of own caseload, as specified by the Service Manager.
Participate in case supervision and case management process.
Maintain and update accurate patient records in accordance with statutory directives. Including maintaining an accurate electronic diary of all work activities.
Communicate sensitive information to patients with dual diagnosis where such patients may be hostile or antagonistic.
Promote mental health for all and combat discrimination against people with mental health/substance misuse problems.
In depth knowledge and understanding of addictive substances, the problems they may cause and the interventions / approaches that help.
Ability to and experience of working within an integrated multi-disciplinary team.
Ability to liaise effectively with and support other professionals who are involved in providing services for people with difficulties related to substance misuse.
To establish and deliver a range of interventions for patients on a 1:1 basis which will provide an effective response in accordance with individual needs and abilities.
Evaluate the effective delivery of care and support through regular discussions, reviews and a collaborative approach to care.
Participate in the development of integrated care pathways for patients with dual diagnosis.
To be responsible for identifying the specific interventions and skills required from the multidisciplinary team to deliver the plan of care and expected health/social outcomes which are evidenced based and lead to recovery and minimise risk for specifically this client group.
To establish and maintain good liaison with GP practices and other mental health and substance misuse services in the area.
