Leaving Care Team Manager will be responsible for providing direction and leadership to a team that delivers successful pathway plans for young people in or leaving care.
The requirements for the post are CQSW, DIPSW, BA HONS SW or equivalent, with 3 years post qualifying experience in a field social work setting, including safeguarding experience.
Key responsibilities will include:
Overseeing a team of Personal Advisors and the development of individualised pathway plans for young people transitioning out of care.
Providing guidance and support to Social Workers and other team members. Ensuring that they adhere to policies, procedures, and best practices.
Advocating for the rights and well-being of young people in or leaving care. This includes liaising with external agencies, schools, and healthcare providers.
Identifying and managing risks related to young people's safety, mental health, education, and housing.
Working closely with other professionals, such as psychologists, educators, and housing officers, to ensure holistic support.
Monitoring the quality of services provided by the team and implementing improvements as needed

- DIPSW or equivalent qualification in Social Work
- Social Work England registration
- Enhanced and in date DBS certificate.
- Excellent communication skills and be able to make good decisions based on information available.
- Can work alone and within/contribute to a team/multi-agency set ups
- Possess knowledge of service needs.
- Competent IT skills and be able to navigate electronic file/recording systems.
- Can write clear, concise, and robust assessment/reports/files.
- Have a flexible approach to work and working hours when the need arises
