While experience in this specialty is welcome, it's not essential. If you bring a different clinical background, we'll support your learning and growth every step of the way in this specialized area( We ask basic EOL or Palliative care aspect has been completed when you did your training)

What You'll Be Doing:
Our Hospice Outreach Team is the first point of contact for new community referrals and incoming calls. You'll be on the frontline, providing crucial palliative advice and support to patients and families in their homes, including care and nursing homes. As part of a rotating schedule, you'll offer telephone support and conduct clinic and home-based assessments, ensuring patients receive individualized care that enhances their quality of life.
We care for individuals with long-term conditions such as heart failure, respiratory disease, frailty, neurological conditions, and cancer diagnoses. Our focus is on patient-centered goals, and we work closely with other community teams and specialists to optimize patient care and continuously develop our team's expertise.

To Succeed in This Role:
You're a motivated clinician capable of working independently while thriving as part of a responsive, resilient team. You have advanced communication skills and excel in both oral and written presentations.

Over the course of this two-year contract, you'll follow a structured framework that will allow you to train, develop, and grow into a Clinical Nurse Specialist or Clinical Paramedic Specialist.
