Carers Practice Advisor

This role has a starting salary of £32,512 per annum, based on a 36 hour working week. Part time applicants are also welcome to apply.

We are excited to be hiring a new Carer's practice advisor to join our fantastic Mental Health North West team. The team is based in Quadrant Court, Woking however it is our intention to move within the local area to Victoria Gate in Central Woking later this year.

Our Carer Practice Advisor position is an exciting and varied role that promotes best practice in whole family assessment and support planning within our Adult Social Care Teams and provides support to the Mental Health social care team in North West Surrey areas including Woking, Spelthorne and Runnymede. The role includes working office and hybrid working.

About the Team

The Northwest Mental Health social care team is a friendly team who work within the Care Act 2014 to assess and support adults with mental health needs and their carers.

About the Role

A day in the life of the role may include carrying our carer assessments or supporting social care staff with assessments, leading 1:1 and group good practice sessions with colleagues, developing and delivering training to small groups within your team, networking with our colleagues in the health sector, carers support organisations and local voluntary sector groups. You will be presenting information sessions to public groups, representing your team at local strategy groups, and supporting a colleague on a complex visit involving a carer.

Other typical tasks will include delivering preventative, early identification workshops at carer support group sessions in our local communities and education settings, reviewing your team's performance against defined carer and young carer performance measures and developing an action plan to deliver improvements and best practice.

You will work alongside your team-based Adult Social Care practitioners as well as our area-based team of other Carer Practice Advisors overseen by our Senior Carers Practice & Development Lead. You will collaborate with them on new ways of working to deliver best practice and improve your teams Carers performance. This is all in line with the Care Act 2014 and our Carers Strategy to really influence change and make sure no one is left behind.

You will work with internal and external partners to raise the profile of both adult and young carers. You will be competent in leading and developing carer's practice within the team and able to support carers in complex situations through finding creative and dynamic support solutions.

Shortlisting Criteria

To be considered for shortlisting for this position, your application will clearly evidence the following:

  • Ability to show an understanding of the circumstances of people with more complex social care needs, to gather detailed and appropriate information and to reach a view about the likely source of assistance
  • Experience of working with carers and young carers in social care context.
  • Working knowledge of Care Act 2014 legislation
  • Ability to work effectively and flexibly as part of a team, and provide guidance and assistance to less experienced or more junior members of staff
  • Able to plan, manage and prioritise a caseload and seek guidance where necessary.
  • Competent in a range of IT tools including MS Office and database management systems
  • Ability to travel across north west Surrey to support carers
