The Acute Frailty Physiotherapist is responsible for providing specialized physiotherapy assessment and intervention to older adults and individuals with frailty syndromes in an acute hospital setting.

The role aims to promote independence, optimize functional mobility, and support safe discharge planning for patients who may have complex health and social care needs.

The physiotherapist will work closely with a multidisciplinary team (MDT), including geriatricians, nurses, occupational therapists, and social workers, to deliver person-centered care and improve patient outcomes.

- Conduct comprehensive physiotherapy assessments, including functional mobility, strength, balance, and cognitive assessments, tailored to the frail and older adult population.

Develop and implement individualized treatment plans, setting realistic and achievable goals in collaboration with patients, families, and caregivers.

Monitor and adjust treatment interventions based on patient progress, ensuring evidence-based and safe practices are adhered to in all clinical interventions.
