Description:

This is a new post within the team, where you will oversee the OT pathways, ensuring a smooth flow for the patient journey. This includes managing occupational, functional, sensory, and reablement pathways.

You will be responsible for managing and overseeing the waiting list for all pathways, assessing patients, and working with support workers under the reablement pathway, providing supervision where appropriate. Your role will be crucial in integrating this workstream within the team to offer a holistic and cohesive approach for patients under our service.

Key responsibilities include:

  • Providing specialist occupational therapy assessments and interventions underpinned by the Model of Human Occupation and the principles of recovery enablement/reablement.
  • Using specialist clinical skills and reasoning to manage a caseload with complex needs, working autonomously and referring to senior staff when necessary.
  • Applying a deep understanding of the effects of mental and physical health conditions on occupational performance; providing training and advice on lifestyle changes and adaptations to the service user's social and physical environment.
  • Collaborating with service users to promote vocational rehabilitation and acting as a team resource/specialist in vocational rehabilitation and employment.
  • Contributing to the delivery of specialist occupational interventions, including sensory integration approaches and enabling/reablement approaches.
  • Ensuring that all care planning involves the service user and, where appropriate, the carer, collaboratively identifying, pursuing, and reviewing goals within the context of care planning.

Skills:

  • HCPC Professional Registration as an Occupational Therapist.
  • Additional clinical training relevant to the specialism.
  • Specialist knowledge of OT assessment, intervention, and practice models relevant to the service user group (MoHO).
  • Knowledge of HCPC professional standards.
