Peterborough City Council operates a reablement service dedicated to helping individuals regain independence in their daily activities. Our goal-based assessments are designed to support clients in achieving their independence, utilizing technology-enabled care devices and occupational therapy aids. We prioritise client interaction while also providing supervision and mentoring to our dedicated staff.

So, what will you be doing as our newest Reablement Practitioner?

  • Conducting goal-based assessments for new clients entering our Reablement Service.
  • Working closely with health and social care colleagues to support client discharges from acute hospitals and rehabilitation for those living in the community.
  • Providing supervision and mentoring to Reablement support workers delivering care and support to vulnerable clients in their own homes.
  • Supporting clients in utilizing technology-enabled care devices and occupational therapy aids to enhance independence.
  • Maintaining knowledge of care practices, triage skills, risk assessment, CQC standards, and the Care Act.

In this role you will be working a 9 week rota with a total of 3 weekends within this. This is a must!
