Join our dynamic Pathways Leaving Care Team in Middlesbrough! We're looking for a passionate Social Worker to support young people transitioning out of care, guiding them toward independent living.
Responsibilities:
- Help care leavers access housing, education, and employment.
- Collaborate with young people to create and review their personalised Pathway Plans.
- Monitor well-being and address any concerns.
- Coordinate with agencies to provide holistic support.
- Manage a varied caseload with efficiency and care.
Qualifications:
- Degree in Social Work with Social Work England registration.
- Experience with young people leaving care.
- Strong communication and multi-agency collaboration skills.
