About the Role
As a Social Worker in our Assessment Team, you will be responsible for conducting thorough assessments of individual's needs and developing tailored support plans
You will:
- Conduct comprehensive assessments of individuals referred to the team, including gathering information, interviewing clients, and analysing their needs.
- Develop individual care plans that address identified needs, taking into account the individual's strengths, aspirations, and circumstances.
- Collaborate with other professionals, including healthcare providers, schools, and community partners to ensure a holistic approach to support.
- Advocate for individual's rights and needs, ensuring they have access to the appropriate services and resources.
- Maintain accurate and up-to-date records, adhering to relevant legislation and policies.
Who we're looking for:
We are looking for experienced and enthusiastic Social Workers who are committed to making a difference in the lives of vulnerable individuals.
You will have:
- A recognised social work qualification (e.g., DipSW, CQSW, BSC Social Work)
- A valid Social Work England registration
- Experience in conducting assessments and developing care plans
- Excellent communication and interpersonal skills
- The ability to work independently and as part of a team
- A strong commitment to ethical practice and safeguarding
