This is a band 6 position available for an experienced health visitor working in the community with vulnerable children and their families.
Start date: ASAP-ongoing
Shift pattern: Monday-Friday, 9:00-17:00
Hours: Full time (37.5hrs/week)
Job description:
- Leading and coordinating the healthy child programme by carrying out the 5 mandated contact visits. Providing a seamless service to universal and vulnerable children and families by working in partnership with multidisciplinary teams. Early identification, intervention, and appropriate onward referral. Good communication skills both written and verbal.
