We are looking for dedicated and experienced social workers to join our Community Adults Learning Disabilities team in South Yorkshire. Job title: Social Worker, Community Adults Learning Disabilities Job Type: Full time, Permanent Hours: 37 hours per week Salary: £31,372 - £41,438 Purpose: To promote and maintain independence for vulnerable adults To protect adults at risk by the use of procedures to safeguard adults from abuse. Together with the service users and carers, formulate and implement personal budgets and support plans Monitor, review and evaluate services and direct payments in order to ensure that they are provided in accordance with need. Responsibilities and Duties: Ensuring the ethos of wellbeing is adhered to when working with vulnerable adults and their carers. Participate in the operation of duty and respond accordingly to need, which will involve Safeguarding enquiries and Best Interest Assessment (BIA) when required. Provide an assessment service to individuals, their carers and other agencies in accordance with Care Act (2014) criteria. Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers, in conjunction with professional colleagues and representatives of voluntary and private agencies. Through the assessment and prioritisation of need, commission appropriate services and authorise expenditure on services within designated limits. Monitor the efficiency and effectiveness of individual care and support plans. Review the needs of individual service users and carers. Respond to and where possible resolve complaints within the Council's complaints procedure. Advise Team Leader on quality and value for money of commissioned services. Participate in multi-disciplinary and inter-agency meetings and working groups and represent the Department as required. Co-ordinate, participate and where appropriate chair case conferences and planning meetings including Safeguarding meetings. Knowledge & Skills: Skills in undertaking strength-based assessments Interpersonal skills to negotiate with others. Ability to undertake safeguarding enquiries Ability to use the mental capacity to determine capacity act to make best interest decisions where appropriate Good team and collaborative working skills. Ability to listen and work with adults in person centred approach. Ability to understand budget management principles. Ability to chair meetings. Demonstrate a sound working knowledge of Social Services legislation and practice. Knowledge of relevant current health and social care legislation relating to adults and young people in transition, e.g. Care Act (20140, MCA (2005). Working knowledge of duties and responsibilities of Health, Public Health, Financial Regulations and Voluntary Sector. Experience: Relevant post-qualification experience in vulnerable adults social work within a statutory, voluntary, or independent sector organisation. Qualifications: CQSW / DIPSW / Degree in Social Work or equivalent.
