To undertake assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
To undertake statutory intervention where service users are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults.
This includes promoting a multi-agency approach by developing and maintaining effective working relationships.
This is a Learning disabilities role in a low security unit so previous experience working with patients who have Learning Difficulties is necessary for a successful application.
The post holder is responsible for the implementation, evaluation and delivery of a specialist dietetic service for Adults with complex conditions that have been referred to the dietetic department across acute, community and mental health services.
Post holder will be a source of expertise to health care professionals on the specialist nutrition support across Adult services
