We are seeking a dedicated Community Enabler to join our team in providing essential support and care services to individuals in need.
The role is to support those that have recently been discharged from Hospital, to provide them with the care they need in order to regain their normal day to day routine, following their recent stay in Hospital.
They are assigned to this NHS community team, for a period of 5-6 weeks, with the goal of gaining a positive outcome after this period of support.
Manage a caseload within a framework of appropriate reflective supervision, undertaking focused, analytical assessments, SMART care plans and reviews that have clear objectives and outcome measures in order to ensure that all looked after children receive a service that reflects the Council's commitment to achieving positive outcomes for our looked after children.
Demonstrate and model good practice that reflects a commitment to relationship and strengths based practice, working collaboratively with children and their families.
Work directly with children and young people to establish their lived experiences, wishes and feelings, using a variety of techniques, as required.
