Hackney Borough Council are seeking an experienced Hospital Discharge Social Worker to join the Out of Borough Team.
The primary responsibility of this role is to facilitate the discharge of Hackney residents who are inpatients at hospitals located outside of the Hackney borough, within London.
Key Responsibilities
Discharge Coordination: Manage and oversee the discharge process for Hackney residents admitted to hospitals outside the borough, ensuring that appropriate care plans are in place.
To undertake statutory social work with children who are Looked After, providing support, assessment of their ongoing and changing needs and interventions which will secure their welfare.
As the Social Worker you will attend any Court hearings as appropriate and give evidence when necessary.
To undertake assessments of the needs of children, their families and formulate plans for those children in accordance with statutory responsibilities and departmental policies.
