We have an exciting opportunity available for an Activity Support Worker to initiate, plan and provide enjoyable and stimulating group and one-to-one activities to clients.
Creative Support is a non-profit organisation which provides high quality person-centred social care services for people with learning disabilities, mental health and other needs.
These activities will be based on the needs, interests and preferences of older people living in our care services in Tower Hamlets.
Work with the service user and their families/carers to develop a package of support/assistance as they prepare to become adults and in their early adult life that promotes and maintains independence and wellbeing; where appropriate involve other agencies and use community resources to provide equality of opportunity, full participation and inclusion in society.
Manage a caseload of disabled children or young people aged 0-25 to assess, plan, monitor and review the support/personal assistance needs of service users, working in a person-centred way.
Main duties and responsibilities
Manage a complex and diverse caseload of children, young people and their families, undertaking assessments to determine the needs and capabilities of clients and carers, and drawing up plans of support under the supervision and guidance of a designated supervisor, using agreed eligibility criteria and in accordance with statutory requirements, Directorate and Corporate Policy and National Legislation.
Industrious Recruitment are currently looking for Support Workers with recent NHS experience or Care home experience, looking for extra shifts to work within a care home setting or in Crisis Centres.
Support Workers/Healthcare Assistants must be well versed in a number of care and medical techniques.
